Health System Impact Fellowships- Continuing Care Branch in Alberta Health New!

Information: Priority Areas (excerpted):
1. Changes to home/community care – in light of proposed expansion and enhancements (including targeted funding from the federal government), there is the need to establish a formalized system for managing, monitoring, and assessing the impact of these changes to home and community care in Alberta. This would include improved public reporting on what results are expected to be achieved and whether we are achieving the planned objectives.

a. Standardized information on the home care population – There will an emphasis on expanding and improving upon information provided to the public and for internal monitoring and planning purposes with respect to the province’s home care client population.
2. Alberta’s Dementia Strategy and Action Plan – Pending its public release, there is the need to develop an overarching implementation framework to monitor the progress made and impacts associated with this provincial strategy and plan.
